Question 1) what kind of closure procedure is used to complete a project?
Before considering a project closed. I would make sure we/ they delivered what was promised. I would run it by the stakeholders for sign off to make sure everyone agrees. Otherwise, no one should get paid.
Question 2) Are Lessons learned used?
I think it started with not clearly defining requirements and not having proper checks along the way. Everywhere I have worked, I have seen an attempt to do a retrospective and document lessons learned. However, I seldom see anyone recall those lessons and use them to avoid mistakes. An example is that we constantly wonder why users are not as happy with the product as the delivery team is. The team thinks the product is good. However, the use thinks that the product is not so good. In my opinion the wrong people were at the table when the gathering requirements. The most important stakeholder was not at the tabled. In this case the most important stakeholder would be the customer/user. An example that I can think of is when we gave the user a system to access their resources wirelessly. The system is now% faster than before.
